A short but thorough medical history form — the same information your physician needs to write a safe, personalized prescription for you.

A licensed physician reviews your intake personally — not an algorithm, not a nurse practitioner. Your kit is customized based on your health profile before the prescription is written.

Why renew?

Prescription medications have expiration dates — typically 1–2 years. An expired kit is a kit you can't rely on when you need it most.

The annual renewal replaces expired medications, refreshes your prescription, and ensures your physician has reviewed any changes in your health status since last year.

Your Port MD+ app subscription also continues — with any updated clinical guidelines applied to your assessment flows.

We're launching first in Texas and expanding state by state as additional physician licenses are established. Join the waitlist and we'll notify you when Port MD+ is available in your state.
If the reviewing physician determines that one or more medications are contraindicated for you, those items will be removed or substituted. In rare cases where the kit is not appropriate, you'll receive a full refund.
Physician review is typically completed within 1–2 business days of receiving your intake form. You'll receive an email when your prescription is approved and your kit enters fulfillment.
Yes. Prescription medications are generally eligible under IRS Section 213(d). We accept HSA and FSA cards at checkout. Check with your plan administrator if you have specific questions about your plan's coverage.
You can update your health profile at any time in the Port MD+ app. Changes that affect your medication safety will trigger a physician review before your next renewal ships.
No. Port MD+ is a physician-prescribed kit for situations where you cannot quickly reach professional care — not a replacement for it. The Port MD+ app is explicit about when to call 911 or seek urgent care. For life-threatening emergencies, always call 911 first.
